Making an Appeal

In order for the process to begin, you must submit an appeal in writing. You can write a letter or you can print off the MSAT Appeal and Consent Form. Fill in the required information and make sure you sign and date the form before sending it to us either by mail or via facsimile at the address below.

If you choose to write a letter, try to include as much of the following information as you can:

Personal information:

  • Your full name
  • Your date of birth
  • Your home Settlement
  • Which Settlement the dispute is on (if different from your home Settlement)
  • Your current mailing address
  • Your phone number
  • Any other contact information (email, fax number,etc)
  • Times that are best to contact you

A description of your appeal, which could include:

  • What event you are appealing;
  • What result you would like to see;
  • Who the disagreement is with;
  • What actions have been taken so far, and on what dates; and...
  • Any letters or other communication with with the other people involved in the dispute (this includes to and from the Settlement).
